Taking these two medicines together can lead to a rare but very serious condition called serotonin syndrome. This happens when there is too much of a chemical called serotonin in your brain and body. Symptoms can include feeling very agitated, confused, sweating, shivering, a fast heartbeat, muscle twitching, or diarrhoea. In severe cases, it can lead to seizures or loss of consciousness.

Why this happens
Both St John's Wort and sumatriptan increase the levels or activity of serotonin in the brain. St John's Wort prevents serotonin from being reabsorbed, while sumatriptan mimics serotonin to stop a migraine. When used together, the effect is 'additive', meaning the serotonin levels can rise to a dangerous level.
What you should do
Back to contentsYou should not take St John's Wort if you are using sumatriptan. If you are already taking both, speak to your doctor or pharmacist before stopping, as they can advise you on how to do so safely. If you experience any symptoms of serotonin syndrome, such as confusion, sweating, or a racing heart, seek medical help immediately.
