Taking ashwagandha with escitalopram may increase the risk of feeling very drowsy or sleepy. There is also a theoretical risk of a rare but serious condition called serotonin syndrome, where levels of the chemical serotonin in the brain become too high.

Why this happens
Both substances affect brain chemistry. Escitalopram increases serotonin levels to treat depression/anxiety. Ashwagandha has calming effects on the central nervous system and may also slightly increase serotonin. When used together, their sedative effects can add up, and their combined impact on serotonin could potentially lead to an over-accumulation.
What you should do
Back to contentsUse caution if combining these two. It is best to speak with your GP or pharmacist before starting ashwagandha while on escitalopram. If you do take both, monitor yourself for excessive sleepiness, dizziness, or signs of serotonin syndrome (such as shivering, sweating, a very fast heartbeat, or muscle twitching). If you experience these, stop the supplement and seek medical advice.

Both medicines increase serotonin levels. Taking them together significantly increases the risk of serotonin syndrome, a potentially life-threatening condition. Seek immediate medical help if you experience agitation, rapid heartbeat, or muscle stiffness.
Hyperthyroidism (Overactive Thyroid)
Ashwagandha can increase thyroid hormone levels. You must not use it if you have an overactive thyroid, as it can worsen the condition or trigger a thyroid storm.
Bleeding Disorders
Escitalopram increases your risk of serious bleeding, especially in the stomach. This risk is much higher if you also take aspirin, ibuprofen (NSAIDs), or blood thinners like warfarin.

You should avoid or strictly limit alcohol while taking these medicines. Both escitalopram and ashwagandha can cause drowsiness and affect your coordination. Alcohol will significantly increase these sedative effects, making you feel more sleepy and increasing the risk of accidents.
Caffeine (Coffee, Tea, Energy Drinks)
Escitalopram can sometimes cause jitteriness or anxiety when you first start taking it. High amounts of caffeine can worsen these side effects. Monitor how you feel and consider reducing your caffeine intake if you feel restless or have trouble sleeping.
