Can I take Levothyroxine and Tramadol together?

There is no direct interaction between levothyroxine and tramadol. However, if your thyroid levels are not well-controlled (if you have an underactive thyroid that is not yet stable), you may be more sensitive to the side effects of tramadol, such as drowsiness or slowed breathing.

Why this happens

Levothyroxine is a synthetic version of the thyroid hormone T4, while tramadol is an opioid painkiller. They do not interfere with each other's absorption, metabolism, or excretion. The only concern is a 'pharmacodynamic' one: if a patient is currently in a hypothyroid state (low thyroid levels), their metabolism is slower, which can make the body process painkillers like tramadol more slowly, increasing the risk of side effects.

It is generally safe to take these two medicines together. Ensure you take your levothyroxine on an empty stomach, usually 30 to 60 minutes before breakfast, to ensure it is absorbed properly. If you feel unusually sleepy, dizzy, or have difficulty breathing while taking tramadol, contact your doctor. Ensure your thyroid levels are monitored regularly as part of your routine care.
