Taking aspirin and ramipril together can sometimes make the ramipril less effective at lowering your blood pressure. There is also a small risk that the combination could affect how well your kidneys work, especially if you are older, dehydrated, or already have kidney problems.

Why this happens
Ramipril works by relaxing blood vessels through a chemical process in the body. Aspirin (especially at higher doses) can block the production of substances called prostaglandins, which help ramipril work and also help protect the kidneys. When these are blocked, blood pressure may rise slightly and the kidneys may have to work harder.
What you should do
If you are taking low-dose aspirin (75mg to 150mg) for heart protection, it is usually fine to take with ramipril, but your doctor should still monitor your blood pressure and kidney function. Avoid taking high-dose aspirin (for pain or fever) regularly without speaking to your doctor. Ensure you stay well-hydrated and report any signs of swelling in your ankles or changes in how often you urinate.
