Can I take Indapamide and Magnesium together?

Drug interaction guide

Taking indapamide and magnesium together can lead to low levels of magnesium in your blood. This is because indapamide, a 'water pill', increases the amount of magnesium your body loses through urine. Additionally, if you are taking magnesium to lower your blood pressure, it may have an additive effect with indapamide, potentially causing your blood pressure to drop lower than intended.

Why this happens

Indapamide is a thiazide-like diuretic that works by making the kidneys flush out extra salt and water. During this process, it also causes the kidneys to excrete more minerals, including magnesium. While you are taking a supplement to add magnesium, the medication is simultaneously working to remove it. Furthermore, both substances can relax blood vessels, which can lead to a combined (additive) blood pressure lowering effect.

What you should do

You should inform your doctor if you are taking magnesium supplements while on indapamide. Your doctor may want to monitor your mineral levels with occasional blood tests to ensure they stay within a healthy range. Watch out for signs of low magnesium, such as muscle cramps, tiredness, or an irregular heartbeat. If you feel dizzy or lightheaded, your blood pressure may be too low, and you should seek medical advice.

Important precautions

Sulphonamide allergy

Do not take indapamide if you have a known allergy to sulphonamide medicines (such as certain antibiotics), as you may have a similar allergic reaction to this medicine.

Severe kidney impairment

These medicines must be avoided or used with extreme caution if you have severe kidney disease, as indapamide will not work effectively and magnesium can build up to toxic levels in your blood.

Low potassium (hypokalaemia)

Indapamide can significantly lower your potassium levels. You must have regular blood tests, especially if you are also taking digoxin or have heart rhythm problems, as low potassium increases the risk of serious heart toxicity.

Food and drink warnings

Alcool

Drinking alcohol while taking indapamide can increase the blood-pressure-lowering effect of the medicine. This may make you feel dizzy, lightheaded, or faint, especially when standing up. It is best to keep alcohol intake to a minimum.

Dairy products and high-calcium foods

Large amounts of dairy products (like milk, cheese, or yoghurt) or calcium-fortified foods can interfere with how your body absorbs magnesium. It is best to take your magnesium supplement at least 2 hours before or after eating large amounts of dairy.
